Physiotherapy Mobility Assessment

Guide Healthcare home care physiotherapists are dedicated to assisting older adults to maintain or improve their mobility. An older person’s ability to remain independent and at home depends not only on their ability to mobilise within the home environment but also to be safe and confident mobilising within their community.

Our home care physiotherapists will complete a comprehensive functional assessment of each client and identify areas for improvement.

Part of this assessment focuses on establishing a person’s individual goals and challenges to create a treatment program specific to their wants and needs. Typical goals include improving a person’s ability to transfer in and out of bed, the ability to access public transport and improving exercise tolerance. 

Following the mobility assessment, a home exercise program is established to assist the person in working towards their goals. Our home care physiotherapists will also consider the need for a referral to other allied health disciplines to ensure the person receives a holistic and comprehensive treatment plan. We know that a multi-disciplinary approach to healthcare achieves the best outcomes for older adults.
