  • Step Stronger

    Guide’s Step Stronger program is a specialised, evidence-based falls prevention initiative designed to keep people safe, mobile, and independent. Through a mix of fun balance activities, lower limb strengthening, and community access progressions, the program targets the key areas that reduce fall risk. Education is woven throughout for both participants and families, ensuring confidence extends beyond the sessions. Step Stronger helps people build strength and balance, boost independence, and enhance overall quality of life.

  • Bright Minds Together

    Guide’s Bright Minds Together program is designed to support the physical, cognitive, and social wellbeing of people living with dementia, along with their care partners. Through tailored exercises, cognitive stimulation, and social connection, the program builds functional strength, confidence, and independence. We create a safe and engaging environment that sparks memories, encourages connection, and brings joyful moments to everyday life. With education woven throughout for families and carers, to support each person and their care network.

  • Returning Home

    Guide’s Returning Home program is designed to support older adults transitioning back into the community after an acute event, such as joint replacement, a neurological event, or hospital stay. Through tailored exercises and wellbeing modules, the program helps rebuild strength, restore mobility, and boost independence. Returning Home addresses the key areas needed to settle safely and confidently back at home.

  • Renewed Diagnosis

    Guide’s Renewed Diagnosis program is an evidence-based, individualised service designed for older adults recently diagnosed with a specific condition. The program helps people optimise daily function, improve mobility, and conserve energy. With a focus on building strength and balance, managing fatigue, and boosting confidence, Renewed Diagnosis empowers older adults to maintain independence and quality of life.

Our Support at Home Services

How does it work?

Submitting a referral for Home Care? Here’s a step-by-step of how the process works.


Step 1 

Submit your referral to Guide here!


Step 2

We’ll contact your client directly to arrange a visit. Don’t worry, we’ll notify you after with a confirmation of initial booking!


Step 3

Your Guide clinician will visit your client and complete an initial report and comprehensive treatment plan - we’ll share this with you within 24 hours.


Step 4

Once the client’s treatment plan is approved by you, we’ll go ahead and schedule all upcoming bookings.


Step 5

Your Guide clincian will keep in contact with you throughout the client’s allied health treatment journey with prompt clinical notes, ongoing treatment plan adaptions, and more!
