Wellness and Reablement Approach
The aged care sector is continuously changing to meet the needs of older Australians. People live longer, have more complex care needs and want to live in their homes for as long as possible. There is now a growing body of evidence that demonstrates the benefits of focussing on client independence. Traditional models of service delivery that focus on what a client can’t do rather than what they can tend to lead to an over-reliance on services by clients, which has been linked with accelerated functional decline. Research suggests that the largest influencer in age-related decline is not genetics but rather lifestyle choices.

The sooner someone stops performing certain tasks for themselves, the faster they tend to lose their functional ability.
The aim of reablement programs is to assist people in performing daily tasks independently for as long as possible so they maintain the ability to maximise independence and autonomy. Our physiotherapy services assist people in retaining or even improving their physical ability, which helps them to continue doing the things they enjoy for longer.

At Guide Healthcare, we recognise that even with frailty, chronic illness or disability, most people want and are able to improve their physical, social, and emotional wellbeing, to live autonomously and as independently as possible. Our wellness and reablement programs allow us to work with older people to maximise their independence and enable them to remain living safely in their own homes and communities.

Guide Healthcare’s reablement programs assist older adults to regain or re-learn, skills required for daily living which have been lost due to deteriorating health or advancing frailty.
Reablement programs are led by Allied Health Professionals and work best when delivered by a multi-disciplinary team. Reablement programs are generally time-limited, usually around 6-12 weeks, with specific goals set around activities required for daily living.
