Pressure Care Prevention and Management

Pressure Injuries are defined as a localised injury to the skin, underlying tissue or both, usually over a bony prominence, as a result of pressure, shear or friction, or a combination of these factors. It’s important to note that pressure injuries are largely preventable. For older adults, pressure injuries can have a serious impact on the person’s quality of life.

Consequences of pressure injuries include:

  • Infections

  • Pain

  • Sleep disturbances

  • Poor mood and outlook on life

  • Delayed healing

  • Financial burden

An older person can be considered at an increased risk of pressure injuries if they are experiencing an acute or prolonged illness or if they are experiencing decreased mobility, impaired sensory perception, previous pressure ulcer and skin conditions, impaired cognition, nutritional deficiencies, poor skin tolerance, disease/illness/surgery, incontinence, and co-morbidities such as diabetes.

Guide Healthcare’s home care occupational therapists take a holistic approach to pressure care prevention and management. We aim to prevent any potential incidence or create an effective management strategy for an existing pressure area.

Our clinicians will go through the process of completing:

  • A comprehensive assessment of the person’s skin integrity and pressure injury risk

  • Provide information and training on appropriate pressure relieving equipment such as pressure-relieving cushions, alternating air mattresses and heel protectors

  • Education on safe manual handling and repositioning strategies

  • Coordinating and communicating with relevant health professionals

  • Follow up assessments and planning for ongoing management of risk
