Medicare Chronic Disease Management Plan

The Chronic Disease Management (CDM) program (formerly EPC) is an initiative by Medicate to assist General Practitioners (GPs) to manage the health care of people with chronic or terminal medical conditions. A chronic medical condition is one that has been (or is likely to be) present for six months or longer. If deemed eligible by their GP, your residents may be able to access up to five bulk-billed allied health treatments per calendar year, including podiatry.

We support the maximisation of funding through the Medicare CDM program in line with legislative requirements.

Guide Healthcare administers this process to significantly minimize the workload for facility staff.  All eligible residents referred to us are bulk billed to Medicare under the CDM program. Our dedicated head office team can provide you with more information about the CDM program and work with you to better understand the process.

We assist in CDM process by:

  • Working with onsite staff to ensure each resident who may be eligible for podiatric treatment has a GP referral in place

  • As per the requirements of Medicare, we complete the first treatment letter to the residents GP which outlines the individual assessment and management plan

  • As per the requirements of Medicare, we complete a summary report after the 5th treatment which is supplied to their GP outlining improvements and ongoing care needs

  • Ensuring our teams and your home are up to date with any changes to the Medicare CDM program
