This decision may have been impacted by an acute traumatic event or a challenging decline in function or cognition. Regardless, the initial phases of entering an aged care facility as well as the ongoing process of care can be dramatically improved by effective communication and the support of on-site staff.
Guide Healthcare clinicians are committed to participating in case conferencing and return from hospital planning sessions. Wherever possible, we want to make sure that we understand the needs and desires of the individual resident and their support network.
In the event that a resident is returning from hospital, your Guide Healthcare clinician can ensure that the resident has the appropriate supports to keep them safe and happy. This may include contacting the discharging hospital to receive up to date information on any changes in mobility or care needs.

Residents rightfully expect that their physiotherapy services will be delivered as per their usual treatment plan. Similarly, it’s critical that service hours are consistent with meeting the requirements of any pain management claims.
We have implemented and continue to build on our layered coverage mechanisms, ensuring that all contracted service hours are delivered. All coverage plans are communicated to on-site management in a timely manner to ensure that there is a sounds understanding of who will be on-site delivering services. We have a dedicated team in our head office who are available 5 days a week to coordinate cover for unexpected absences or arrange short-term coverage.
Guide Healthcare can also assist with coverage of your permanently employed clinicians. We offer these types of coverage services at our standard rate to all of our partner homes.
